Attention,

5th Grade Parents!

The following information is very important regarding immunization requirements for the 2016-2017 school year per Arizona School Immunization Law, Article 7, R9-6-701-708:

All students are required to show proof of chickenpox (varicella) immunization or to provide a signed history form stating that the student has had chickenpox.

For the 2016-2017 school year, Arizona School Immunization Law states that all students 11 years of age and older entering the sixth, seventh and eighth grades must have one dose of meningococcal vaccine (MV). These students are also required to have one dose of Tdap vaccination if five or more years have passed since their last tetanus/diphtheria vaccine dose.

If your child has received the required immunization, please provide the school with documentation (shot records).  The record must show the child’s name, date of birth, date dose received, and physician/health agency that administered the immunization.

If your child has NOT received the required immunization, please contact your physician/healthcare provider or immunization clinic.  Information on free immunization clinic sites may be obtained from the school health office.

The schools have advised us that schedules may not be given to students who are not in compliance with vaccinations.

As soon as your child has received his or her immunizations, please bring a copy of them to the health office.
